| I, the parent or guardian, understand that
the Indianapolis Premier Soccer Club, does not carry insurance covering my child’s
participation. I accept financial responsibility for any injury, which may occur
resulting from my child’s participation. Furthermore, I agree to hold harmless
and indemnify the directors, coaches, referees, or any assigned officers against
any and all litigation that may arise out of my child’s participation in this
soccer program. I also understand and have read the Indianapolis Premier Soccer Club Code
of Ethics (link to code provided above). I also give the Indianapolis Premier Soccer Club
the authority to utilize my child's picture and name on the club's
website. |
